First Stage of Labor The first stage begins when you go into labor and ends when your cervix is completely dilated to 10 centimeters. If this is your first baby, you may be in this stage for 6 to 12 hours (average) up to as long as 20 hours. If this is your second or third baby, this stage may be shorter. The first stage of labor includes 3 distinct phases:

• Early • Active • Transition

PHYSICAL CHANGES

• Contractions are mild and somewhat irregular but getting stronger and closer together • Contractions may begin 15 to 20 minutes apart and get to 5 minutes apart • Contractions last 40 to 60 seconds toward the end of early labor • Contractions may feel like aching or low backache, menstrual cramps, pressure, or tightening in the pubic area • Amniotic sac may rupture; you may see a pink vaginal discharge (bloody show)

• Excited, eager, anticipating the start of labor • Some fear and anxiety: No turning back now • Wondering if you will remember what you learned in class • Talkative, very social, ready to talk and interact with partner • Fully aware of surroundings, engaged, eager to report symptoms
